Patient Information

For your first appointment:

• You will be emailed an invitation to create an account to your patient portal through Charm Health. Once logged in you will be able to electronically complete your intake form and consent form.

• Please bring laboratory reports, diagnostic imaging reports, etc. that you have available to your appointment. If you don’t have them you may ask your physician to fax them directly to the office at 206-512-3161 or we may request them at your first appointment.

• If you are taking several supplements or medications bring them in or bring in photos of the bottles so we can review them together.
